To date, the Schengen Agreement has been signed by 26 European countries, and each of these countries has the right to issue Schengen visas to foreigners. The rules for obtaining a Schengen visa for each individual country may differ slightly, especially in terms of the documents provided, the procedure for completing them, etc. However, there are no disagreements on the issue of filling out a Schengen visa application form. Therefore, today we will look at how to fill out the Schengen application using the example of a short-term visa to Spain application.

Please note that each country has its own visa application form. If you are applying for a visa, for example, to a German consulate, then the application form must also be for the German consulate. We would also like to draw your attention to the fact that some countries accept for consideration only questionnaires filled out in electronic form. These countries include Poland, Lithuania, Estonia, Latvia, Finland, Slovakia, Sweden, Norway. They have their own rules for filling out questionnaires.

The first and most important thing that you need to learn - the questionnaire is filled out only in block letters of the Latin alphabet. All information provided by you in the application form must exactly correspond to the data in your passport and other documents that you provide to the embassy. Well, let's get started.

Items 1-3 filled in strictly in accordance with the passport. Please note that the patronymic in the Latin alphabet is not indicated in the passport, therefore it is not necessary to indicate it in the application for a Schengen visa.

Point 2 fill in only by citizens who have changed their surname. Here you must indicate all the previous surnames that you had separated by commas. If you have never changed your last name, just leave this item blank. Once again we repeat NULL, i.e. do not write anything at all.

Item 4... You must indicate your date of birth. Pay attention to the date format. In the Spanish questionnaire "day-month-year", but, for example, in the application form for a visa to Germany, the date format is "year-month-day". Read this paragraph carefully.

Clause 5. Place of birth must be indicated. You can write the name of the settlement where you were born, or the name of the region, for example, Gomel region.

Item 6... Indicate the country of birth. See what is written in your passport.

Clause 7... Indicate your current citizenship. It is also necessary to indicate the citizenship at birth if it is different. If not different, then leave the item blank.

Clause 8... Specify gender by ticking the appropriate box.

Clause 9... Indicate your current marital status. Please note that those who have already been married, but are currently free, must indicate "divorced / divorced". “Single / not married” is indicated by those who have never been married.

Clause 10... This section is filled out only if the questionnaire is for a minor applicant. The details of the parent or legal guardian are entered here. If the parent or guardian resides at a different address from the minor applicant's address, then this section must include the adult's address.

Clause 11... Passport identification number (14 characters) is required. This item is optional. You can skip it.

Clause 12... Choose the type of your passport. In our case, it is an ordinary passport.

Clause 13... Indicate the number of the travel document. Please note that the number in the Belarusian passport consists of numbers and letters. Indicate all of them.

Points 14-15... Indicate the date of issue and the validity of the passport. This information can be found on page 33.

Clause 16... Indicate the authority that issued the passport. MINISTRY OF INTERNAL AFFAIRS is indicated in Belarusian passports.

Clause 17... Indicate the registration address. This point is especially important if there are several consular offices of this country in Belarus. For example, Lithuania has not only an embassy, ​​but also a consulate on the territory of the Republic of Belarus and the reception of citizens is carried out on a territorial basis.

Be sure to indicate the phone number in the international format: +375

Clause 18... To be completed only by non-citizens of Belarus. If you are a citizen, then put a tick in the "No".

Clause 19... You must indicate your profession. Specify the profession in English, i.e. not by transliteration, but translate your position into English and enter the correct version. For example, not MORYAK, but SAILOR.

Clause 20... You must provide information about your employer. Please note that proper names are not translated, but are written in transliteration. For example, if you work at OJSC "BEREZASTROYMATERIALY", write to OJSC "Berezastroymaterialy".

Students and schoolchildren indicate the address of the educational institution, and the unemployed, housewives, retirees and preschoolers - the address of residence and contact phone number.

Clause 21... It is necessary to indicate the type of visa requested. You just need to check the box next to the one you want.

Clause 22... Indicate the country of destination, i.e. the country in whose consulate you are applying for a visa. In our example, this is Spain.

Clause 23... Indicate the country of first entry into the Schengen area. If you have a direct flight, then indicate Spain in this paragraph. If you are driving through Lithuania, then indicate Lithuania. If you are flying with a transfer, for example, in Warsaw, then indicate Poland.

Clause 24... It is necessary to select the multiplicity of the requested visa. You can always apply for a multiple entry visa regardless of your visa history. The consul will decide for himself whether to extradite or not. But, as they say, demand is not hit in the nose, so we ask for a multiple-entry visa.

Clause 25... It is necessary to indicate the number of days of stay. If you receive a visa by invitation, then indicate exactly as many days as indicated in the invitation. If you are requesting a visa for a year, then enter 90. You cannot request more than 90!

Clause 26... All previous Schengen visas for the last three years must be indicated. Only for the last three and only Schengen. The rest will not be taken into account.

Clause 27... Indicate whether you have taken fingerprints. If you handed over, put "Yes" and, if possible, the date of delivery (if you remember). If you don’t remember the exact date, just put “Yes”, and do not write anything in the date column.

Clause 28... To be completed only by applicants applying for a transit visa.

Points 29-30... Indicate the dates of entry and exit from the Schengen area. Please note, if you are applying for a multiple-entry visa, then in the column "date of entry" indicate the date of the beginning of your trip, and in the column "date of departure" indicate the expiration date of the requested visa.

For example, you are requesting a visa for 3 months: clause 29 - "12/20/2016"; p.30 - "03/19/2017"; p. 25 - 30 days or 45, no more!

You are requesting a visa for 6 months: clause 29 - "12/20/2016"; p.30 - "06/19/2017"; p. 25 - 90 days maximum!

You are requesting a visa for 1 year: clause 29 - "12/20/2016"; p.30 - "12/19/2017"; p. 25 - 90 days maximum!

Clause 31... To be completed by applicants who request a visa without an invitation. Here you indicate the contact information of the hotel in which you are going to stay during the trip.

Item 32... To be completed by applicants who apply for a visa by invitation. The details of the inviting person are indicated here.

Clause 33... Indicate who finances the trip. Check the box next to the one you want.

Paragraphs 34-35... To be completed only by those who have relatives in the EU. If there are no relatives, just skip.

Item 36... Indicate the place where you will submit documents. There is no Spanish consulate in Minsk, so we indicate Moscow. If you submit documents in Minsk, then indicate Minsk. Do not forget to include the date of signing the questionnaire.

Clause 37... Put your signature.

An additional signature must be put in three more places, in total, you must sign the application for a Schengen visa 4 times!
